First Visit

Periodontics and Dental Implants FLAfter reviewing your medical and dental history forms, Dr. Cohen will complete a thorough evaluation of your radiographs, along with an intra and extra oral examination.  A detailed explanation of his findings will be discussed with you.

Typically, we do not start treatment on the same day.  Usually some preparation is required and a consultation with your treating dentist is performed, so that your treatment is completed with a team approach.  We would also like you to have sufficient time to decide the best treatment option and to ask all the questions you may have.

There is a fee associated with your first visit.  Should you have any questions, please feel free to contact us. 

Preparation for your First Visit

Please complete your patient registration form and bring it with you to your first appointment, as this will shorten the duration of your time in the office.  It is available on our website to download and print in the section titled Online Forms.

It is important that you list any medications, allergies or other pertinent information that could affect your dental care.
Please make sure to provide us with the appropriate dental x-rays (should your referring dentist have given them to you), as they will be needed during this first appointment.  Should no x-rays be available, we will either need to complete a series of x-rays or reschedule you until they are available to us.  On some occasions, even if you have provided us with x-rays, we may need to take additional ones for periodontal / implant diagnostic purposes.

Please continue to see your general dentist, as needed, while undergoing treatment at our office.  It is important that your restorative needs continue to be addressed.  If you have been referred by your general dentist, we will work with them to gather all the information needed prior to your first visit.
